Pelvis X-Ray AP

Pelvis Anteroposterior STD
Objective

Hip joints and pelvic structures.

Positioning

Patient supine.
Lower limbs internally rotated 15–20° (feet together, toes pointing inward) to bring the femoral neck parallel to the detector.
Central ray: perpendicular to the IR.
Correct alignment.

Centering

Pubic symphysis

DR Parameters

75–85 kV · 20–30 mAs
SID ~100–115 cm · grid required

CR Parameters

75–85 kV · 30–50 mAs · SID ~100–115 cm

Quality criteria

Complete visualisation of the pelvis.
Hip joints visible.
No rotation.
Obturator foramina symmetrical bilaterally (confirms absence of pelvic rotation).
Lesser trochanter not visible or barely visible laterally (confirms correct 15° internal rotation).
Sacrum centred relative to the pubic symphysis.

Common errors

Patient rotation.
Incorrect centring.
Movement.

Practical notes

Check for metallic objects.
Routine gonadal shielding is no longer recommended (AAPM 2021); apply dose optimisation according to the local protocol.
Verify compliance with national DRL for this projection.
